Grooming Release Form

YYour pet is very important to us! We would like to assure you that we will always do everything in our power to assure their safety. Safety is our upmost priority for both our team members and your furry friends. Please complete each portion of this release form before your pet's first appointment. Be sure to check all boxes below each paragraph. This release form covers all pets that you currently have and any you may own in the future.

Aggressive Dogs:

Bella & Bow's has the right to refuse services at any time. Aggressive dogs are a risk to both themselves and the groomer. I agree to make the groomer aware of any aggressive issues or biting incidents involving my pet. I understand that for everyone's safety, my pet may need to be muzzled in addition to appropriate restraints. Muzzling is our last resort. I understand that grooming will occur at the groomer's discretion. If at any time the groomer feels that the pet is a safety threat, grooming will cease, regardless to the degree of completion. The pet owner is responsible for the entire cost of the groom.

Shaving Thick Coated/Double Coated Dogs:

Shaving double coated dogs, including, but not limited to, Golden Retrievers, Shepherds, Labs, and Cats, can result in long term and irreversible damage to their coat. They coat may grow back patchy, coarse, or not at all. We will shave any pet at the customer's requests. However, we are not responsible to any long term damage of the coat.

Pet's With Fleas & Ticks:

When a pet is known to have fleas, we take precautions to protect all pet's on the premises. If we are made aware of a possible flea infestation, we will schedule your pet accordingly and take them straight to the bath. Your pet will be given a flea bath and possibly a Capstar at the owner's expense ranging from $5-$15. This should kill all of the fleas on the pet but will not protect it once it goes home.

Matted Pet Policy:

Pet's with matted coats require extra care, time, and attention. It also causes extra wear & tear on grooming equipment. If the matting is minimal and the pet behaves well, we may be able to brush out the mats for an extra fee. However, if the matting is severe, the least painful and least stressful way to remove the mats is by shaving the pet. Bella & Bow's stands against causing serious or undue stress and pain to your pet by dematting heavily matted coats. There is greater risk of nicking or scratching a matted pet during the grooming process. In addition, the skin may appear red, itchy, and/or irritated due to the lack of oxygen reaching in and under the mats. A hematoma is also a possibility if the mats have cut off the blood supply to the tips of extremities (such as the ears or tail). Bella & Bow's will not be held responsible for any injuries sustained while grooming a matted pet, including any after effects. This includes, but is not limited to, swelling, redness, itchiness, hematomas, and/or self-inflicted abrasions due to extreme rubbing.

Veterinarian Release and Filing Agreement:

I (the pet owner) understand that grooming incurs certain risks. Dogs are often distracted and unpredictable. Nicks, scrapes, or other injuries are often unavoidable, especially if one or more of the above conditions apply. As the owner of the pet, I (the pet owner) take full responsibility for the condition and behavior of my dog. I give Bella & Bow's the right to use vet services, should the need arise. Furthermore, I agree to pay all vet bills in full.

All pet's are STRONGLY recommended to be up to date on all vaccinations and are REQUIRED to be up to date on their Rabies vaccination in order to receive any service.

Cancellations / No Shows / Late Policies:

We will make every effort to contact each client before their scheduled appointments to remind them. However, the customer takes full responsibility for arriving to their appointment on time. Appointments are taken throughout the day and are only available for your pet at the specific time you have reserved. If you arrive 10+ minutes after your scheduled appointment time, we reserve the right to cancel your appointment and reschedule you for another day or possibly only do a service that requires less time. If we are still able to provide the service for your pet, there will be a late fee added to your balance of $10.

If you do not show up or cancel your appointment within 24 hours, you will have a $10 fee added to your pet's next visit in addition to normal grooming costs. To avoid these fees, please give us a minimum of 24 hour notice, which will allow us to fill your pet's appointment spot with someone else.

If you are a habitual no show/cancellation offender, you may be asked to prepay for your service appointments. Your pre-payments for any future appointments are non-refundable.

This release form authorizes Bella & Bow's to take photos of your pet and permission to post to our business social media pages/website.
